After attempting an teleport that fails (for whatever reason), all subsequence attempts at teleporting fail. A logoff is required to restore TP functionality.

Carl, if the issue happens again, please comment on your full computer environment Help > About Second Life and before you restart Second Life please attach your latest SecondLife log and crash report to this issue.
