1. Open the inventory by clicking the Inventory button on the bottom right corner of the client window
2. Scroll up or down to a folder that contains all of the necessary parts for a complete avatar change, including a skin textures, objects, huds, gloves, body shape, etc.
3. Right click the folder itself and on the pop up menu click on Replace Outfit at the very bottom.
4. Your avatars objects are taken off and replaced with the objects in the avatars folder. When this is done either the top or bottom texture of the avatar will sometimes be missing. The pictures included in this report show pictures of 3 avatars I have in which this happens.

Spoke to Rabid.

Removing the gloves fixes the problem on upper body.
This is a duplicate of VWR-7957 (Clothing skinning error on nVidia):

It looks likely that you have an NVIDIA graphics card with version 177, or later, drivers.

The following workaround has been reported:

  • Enable the Advanced Menu (Ctrl-Alt-D)
  • Disable rendering of Palletized Textures (Ctrl-Alt-F7, or Advanced/Rendering/Features/Palletized Textures)
  • Rebake (Ctrl-Alt-R)

This bug is fixed in 1.22 RC1 (available from http://www.secondlife.com/downloads)
You can also revert to NVIDIA 175.19 drivers to see if this resolves your issue.
